Experience

August 2010 – Currently

Queen Hospital King’s Lynn NHS Trust.

Occupational Therapist

  • I have been recently working in a specialised community rehabilitation team that covers the Downham Market region. We provided rehabilitation care and a falls prevention service for the wider community. The multidisciplinary team included physiotherapists, occupational therapists, four rehabilitation assistants and a social worker.
  • The job consisted of screening a large case load of candidates who were appropriate for the referral for our services. The rehab treatment provided for the clients was given either in their homes or on the rehabilitation ward within the Trust’s hospital. Clients ranged from adults with physical disabilities, those with various medical and physical needs prior and post hospital discharge.
  • I also worked with adults with complex co-morbidities with some of these clients requiring end of life care. I provided valid assessment rehabilitation and intervention to clients within the Trust’s hospital, client’s home. The rehabilitation assessment included equipment prescription, home adaptations, ADL retraining, comprehensive home programmes, client education including mobility and transfer training. I also provided specialised fall prevention assessment and treatments to clients.
  • Goal setting and treatment planning were monitored and reviewed regularly by myself and the team I worked within. This was achieved by including the carers of the clients by offering assessments for the correct use of manual handling equipment via training. I worked closely with community services, various therapists, rehabilitation units and care facilities during this employment. I also gave valuable leadership and guidance to the rehabilitation assistants.
